Who can observe?
Midwest HealthStrategies welcomes registered observers who are in High School sponsored programs, University graduates applying to therapy programs or anyone who is a freshman in high school or older that is interested in finding out more about our services.
What do I have to do to start?
- Read Observation Requirements
- Contact Student Coordinator with your request to obtain approval to continue with Registration Process.
Acceptance and assignment locations are determined by the Student Coordinator according to availability. Observation is first come-first serve. You will have a time limit of 2 months from your initial request to complete the registration. Your request will be cancelled and opened to other students if incomplete.
What do I have to do once approved to continue with registration?
- Read Observation Info & Registration Instructions
- Complete and submit to the Student Coordinator the following forms:
- Provide Immunization Records to Student Coordinator
- Provide Results from 2 Step TB Tests Student Coordinator (Be sure to read instructions for definition of 2 Step TB test)
- Read and understand Dress Code Policy
Registration Documents and Records must be provided by Deadline ( 2 weeks PRIOR to your expected start date )
*Minimum requirement is 30 hours observation.
How do I track my hours?
Observation Hours must be tracked on the MWHS Time Log and submitted to Kelli Flaming on your last day. (For your convenience, you may ask the assigned location receptionist or therapist you are observing to intermail or fax it.)
Who processes my request for University program application references?
All University forms are to be submitted directly to Kelli Flaming. The required records for accurately processing your request are saved for 3 years.
